Care Plan
A care plan is a individualized written plan developed by our Director of Patient Services (DPS), the patient, the patient doctor order and family members. Each care plan will reflect medical orders provided by the patient's primary care physician (PCP) and state all the patient personal care needs as well as daily chores/activities. All care plans are essential because provides the caregivers and other health professionals with detailed task that must be completed each shift. Adjustments will be made to the care plan on an as needed basis by the DPS.

Private Duty Nursing    
LPN's and/or RN's will provide minor wound care, injections, catheter changes, health observations, medication management and other relative services according to the patient medical order provided by their doctor, rehab/nursing facility or other referral source. 
Home Health Aides
Under the supervision of our Director of Patients Services and according to the patient care plan, the Home Health Aides (HHA) will carry out the following; monitor physical and mental conditions, record intake and outtake of fluids, exercises, blood sugar and blood pressure monitoring, range of motion techniques, transfers,light housekeeping, meal preparation and other relative/supportive ADLs or personal care necessities.

Personal Care Aides
Under the supervision of our Director of Patient Services and according to the patient care plan, the Personal Care Aide (PCA) will carry out the following; light housekeeping, cooking/meal preparation, running errands, laundry, oversight/supervision, transfers, assist with bathing and showering, grooming, assistance with toileting and other relative/supportive ADLs or personal care necessities.

Companions or Homemakers will be provided to patient's who does not require any medical care. This service is not under doctor orders but is directed by the patient or the patient family members preference. Services may include; assistance with daily routines/activities, meal preparation, laundry, paying bills, direction in managing and maintaining their household, socialization, companionship and assistance with maintaining a safe and healthy environment.

Nurse Supervision
All of SHASIA's health care professionals will be supervised by the agency Director of Patient Services (DPS). Annually, the DPS will visit the patient's home to observe the caregiver performance of his/her duties according to the written care plan. The DPS will also monitor the patient health condition and review updated information as it becomes available. The main goal is to keep the communication between the patient, caregiver, doctors and family members strong ensuring quality care.

Respite Care

Respite Care is planned short-term or temporary care provided to families of a disabled children or adults.

Consumer Directed Personal Assistance program

"CDPAP"  is a Medicaid program that provides an alternative way of receiving home care services, where the consumer has more control over who provides their care and how it is provided.  

Emergency Care

Emergency Care is in-home service that helps to individuals who needs a care provided by a caregiver immediately.

SHASIA, LLC is now accepting the following method of payments; Medicaid through MLTC ( Managed Long-Term Care) companies, Oscar health insurance, Carecentrix and private pay.
